Program details:  

This program provides access to an Outreach Worker (Community Facilitator) who will work with the individual to attain their goals. The aim of this program is to provide immediate, brief support to individuals with an ABI (Acquired Brain Injury) within the community. This support differs from the Mainstream Outreach Program such that the individual is not assigned to nor followed by an Outreach Worker for ongoing support– this support is intended for short-term goal completion. 

The program runs consistently every Tuesday through the Day Program located at 450 Westheights Drive in Kitchener from 10:00am until 3:00pm. 

The program can be accessed in-person or virtually, please contact the intake department to book a time.
