ABI Intensive Case Coordination and System Navigation

ABI Intensive Case Coordination and Occupational Therapy support at Traverse Independence can provide community-based case coordination, system navigation and occupational therapy supports for individuals who have an ABI.  This can be assistance with connecting to additional resources and appropriate community supports, including ABI resources across Ontario

Intensive Case Coordination works to provide linkages and collaborations with other service providers to ensure continuity and coordination of services. Case coordination works in partnership with community organizations, primary care, mental health, addictions, housing providers, and all other service providers that assist in addressing the needs of this complex population. 

Many of the clients who require the support of intensive case coordination have significant and complex barriers. The Intensive Case Coordinator is skilled in system navigation from an acquired brain injury perspective. Each client is provided with a personalized, flexible, and individualized care plan, which most typically is short term in nature.

Intensive Case Coordination can assist individuals who have an ABI to learn new skills, can provide support and strategies to optimize their quality of life.Case Coordination and Occupational Therapy can provide functional support while waiting or are receiving Outreach and Transitional Living Services.

Kendra has been an occupational therapist since 2015 after having her sights on becoming an OT since high school.  She completed her undergraduate studies in Kinesiology at the University of Waterloo and minored in psychology followed by her Masters in Occupational Therapy at Dalhousie University. Since then, she has gained an array of experience working in adult home care, motor vehicle accidents, private pediatrics and adult populations.  Her training and experience have significantly focused on assessing and supporting individuals with ABI, neurodivergence, persistent pain, and mental health challenges. She is passionate about social justice and equity and promoting quality of life.  In her spare time, she loves to run, cook (and eat of course!), do yoga, and spend time with family and friends.
