ABI Mainstream Outreach

Traverse Independence offers mainstream outreach services where the staff person, who is a community facilitator provides support in your housing location.

In some cases, ongoing weekly support is needed, in others, just monthly check-ins. Your outreach worker can assist you with things like:

  • Medication reminders, systems for managing medication, support with pharmacy
  • Accessing ABI specialized supports like occupational therapy
  • Connection with other community supports such as mental health and/or addiction providers, the justice system, etc.
  • Time management, organizational skills
  • Banking budgeting and money management
  • Menu planning, shopping, cooking
  • Support and advocacy during medical, legal or other appointments.
  • Training on household tasks (memory aids, check lists, assistive devices, and more)
  • Transportation training

Land Acknowledgement

We acknowledge that we are located on the Haldimand Tract, traditional and unceded territory of the Anishnaabe, Haudenosaunee and Neutral Peoples. It encompasses 980,000 acres of land that was given to the Six Nations in 1784, 10 kilometers on each side of the Grand River.
