ABI Mainstream Outreach
Traverse Independence offers mainstream outreach services: our community facilitator works with you and your family in your home.
In some cases, ongoing weekly support is needed, in others, just monthly check-ins. The community facilitator works with your community-based rehab team to apply goals and strategies to daily living in the following areas of independent living:
- Scheduling appointments and transportation
- Budgeting and money management
- Menu planning, shopping, cooking
- Medical appointment attendance and support
- Implementing rehabilitation programming
- Household tasks
- Transportation training
- Other supports as required
