Supportive Housing
Traverse Independence provides supportive housing and personal support services for adults with physical disabilities. In the Supportive Housing program, you live in your own apartment and are responsible for your own rent and living expenses. You have access to support services 24 hours a day/7 days a week. Services are scheduled on a daily basis with additional on-call support as required. Services may include Attendant Service Workers, Community Facilitators (CF) and Behavioural Facilitators. CFs can support you in many areas, including money management, personal care, and interpersonal relationships.
Attendant services may include, but are not restricted to:
- Diabetic care/medications
- Prescribed exercises
- Tube feeding
- Assistance with dressing or undressing
- Bowel and bladder routines
- Ventilator care
- Bathing and grooming
- Light homemaking and meal preparation
Eligibility Criteria
To be eligible for our supportive housing services, you must:
- Be at least 16 years of age.
- Be a Canadian citizen or landed immigrant and have a valid Ontario Health Card.
- Have a permanent physical disability.
- Provide verification of disability through medical documentation from a valid regulated health practitioner.
- Have any medical issues stabilized with ongoing clinical supervision.
- Not be dangerous to themselves or others or have behaviours that will be disruptive to effective service provision.
- Not have ongoing psychiatric or substance use issues that will be disruptive to effective service provision.
- Have a free will commitment to rehabilitation and/or participation in programs.
- Be able to pay for any costs associated with program participation.
- Be able to determine the support you require and request assistance as needed (direct your own care).
- Require a minimum of 3- 6 hours a day of personal support with 24-hour on-call for assistance between scheduled visits.
- Be safe when left alone.
- Be able to secure and maintain tenancy under the Tenant Protection Act of 1997.

Land Acknowledgement
